COVID-19 vaccination for children aged 12 to 15 years of age

There will be vaccination clinics available across the district during half term for those who missed their vaccination in school, those who had not yet decided on vaccination and for those who have recently turned 12 years of age.
If you have already submitted a consent form for your child, you do not need to complete another one.

To book an appointment, please call the team on 01244 397470. There are a limited number of appointments available, please contact us as soon as possible to book.

When you have booked an appointment, you will be requested to complete the consent form online if you have not already submitted a form.

The attached leaflets and links below will provide you with all the information you require to ensure you are fully informed when making the decision about consent for vaccination.

If you have any queries, regarding specific clinical concerns or the vaccination being given in school, please call 01244 397470 to speak to the School Aged Immunisation Service. They are trained to note down your concerns and will liaise with a clinician to contact you to respond to any issues you may have.

Only children who are aged between 12 years and 15 years old on the day of the vaccination are eligible for the COVID-19 vaccination through our programme. Please do not consent to vaccination if your child is NOT aged 12 to 15 on the day of vaccination.
